- Request an appointment
- Request a prescription refill
- Access immunizations
Need a Login to the Portal?
If you are a new patient and need login credentials for the patient portal, please contact our office.
- Give our front desk staff the email address where you would like portal emails sent.
- You, the parent, will receive an email with a temporary password and link to portal website.
- Once on the portal website, you will create permanent password.
